Before your initial appointment, you will be asked to complete a food diary, comprehensive health history, and lifestyle questionnaire. This information allows your registered dietitian to thoroughly review your nutritional needs, health goals, and medical background in advance, ensuring that your appointment time is focused, personalized, and productive.

Initial Assessment Consultation
Duration: 60 minutes
Your initial assessment is a comprehensive consultation designed to help us understand your unique health journey, nutrition needs, and lifestyle. During this session, we will review your medical history, eating patterns, lifestyle habits, symptoms, and any relevant health concerns. We will discuss your goals and identify the factors that may be impacting your health and well-being. Based on our discussion, we will develop realistic, individualized goals and determine a plan of care that aligns with your needs.
Follow-Up
Duration: 30 minutes
These sessions are essential to guarantee success, monitor progress, review food records, modify meal plans as needed or answer any questions. I will continue to support, listen, and guide you toward your long-term goals. Follow-Ups are recommended every 1-4 weeks, depending on your goals and diagnosis.

I accept some insurance plans and will file insurance claims on your behalf. A claim will be filed with the insurance company after your visit. Even if you have one of these insurance plans, coverage is not guaranteed, so it is recommended to call your insurance company and ask the following questions:
- Does my plan cover CPT codes 97802 and 97803 with Registered Dietitian, Agnieszka Depinska?
- Do I have coverage for telehealth for these CPT codes?
- If they say yes, ask if you/your child must have a certain diagnosis for coverage.
- Do I have a deductible?
- Do I have a maximum allowable number of visits per calendar year?
Out-of-Network Benefits
If I am not in-network with your insurance plan, you may still be eligible for out-of-network reimbursement. Upon request, I can provide a superbill for you to submit to your insurance company. If your insurance does not cover nutrition services, or if you prefer not to use insurance, please contact me for current self-pay rates and payment options.
